Referring Physicians

Referring physicians can expect transparency, open communication, and prompt follow-up from the care team at Mile High OBGYN + Midwifery, including visit summaries, clinical notes, test results, and other important documentation.

Records Release Request for Patients

Mile High OBGYN + Midwifery is deeply committed to protecting your privacy. If you are a referred patient, we may ask you to authorize another physician’s office to provide us with your medical records. This is to ensure that we have all the information needed to provide quality, personalized care. To authorize release, we require all patients to complete and sign a records release form.

This form can be printed at home, completed, and submitted to your outside physician’s office. This form grants them your permission to release your records to our office. They may ask you to hand-carry them to our office. In the event they mail or fax your records, please check with us to make sure the records have arrived in our office before your appointment.
